In consideration of being allowed to participate in the activities and programmes of ODFC Fitness and to use the facilities and equipment owned and/or under the control of ODFC Fitness, in addition to the payment of any fee or charge, I do hereby waive, release and forever discharge ODFC Fitness from any and all responsibility or liability for injuries or damages resulting from my participation in any activities or my use of equipment or facilities in the above mentioned activities.
I understand and I am aware that strength, flexibility and aerobic exercise, including the use of equipment, in the outdoors, are potentially hazardous activities. I also understand that exercise and fitness activities involve a risk of injury and even death, and that I am voluntarily
participating in these activities and using equipment and facilities with the knowledge of the dangers involved. I hereby agree to expressly assume and accept all and any risks of injury or death.
I am aware that I have the right to request advice from any of ODFC Fitness staff, at any time, in relation to the activities and exercise being undertaken and, but not exclusively, their suitability for me, with particular regard to my health and clothing. If I choose not to take advice, or to disregard any advice so given, I do so voluntarily and accept liability for all resulting injuries or damage.
I do hereby declare myself to be physically sound and suffering from no condition, impairment, disease or infirmity or other illness (other than those declared on the attached medical questionnaire) that would prevent my participation or use of equipment or facilities except as
herein stated. I acknowledge that I have either had a physical examination and have been given my doctors permission to participate, or that I have decided to participate in activity and use of equipment and machinery without the approval of my doctor and do hereby assume all responsibility for my participation and activities, and utilisation of equipment and machinery in my activities.
In addition ODFC Fitness cannot accept responsibility for valuables left with the instructors or their vehicles.
Covid-19 Responsible Response & Changes to Keep you Safe
Pre Session Admin
You must be clear of infections and health issues that may affect you training at the start of training. If you become unwell in the coming months, please let us know at the earliest opportunity. This must be before you turn up to the session in person. You can not train at any level if you get ANY symptoms associated with Covid-19. You should follow the latest government guidance in terms of self-isolation.
Due to government guidelines and the public liability insurance we hold, we are unable to train people who are classed at a high risk with regards to Covid-19 and are therefore in the shielding category or anyone who lives with such a person. If you are, or become, a person under this category, training with ODFC Fitness must pause until we are given guidance that it is safe to resume. You must inform me of such a circumstance prior to turning up in person to a session.
Pre-Session Arrival and Departure
All sessions are pre booked using the app “Target Tracker Pro”. You must book onto a session prior to attending.
All sessions are paid for monthly, in advance on our website, via our online DD set up portal. We can take first month payment via bank card or bank transfer. No cash is taken at sessions.
Arrive at your pre booked session location promptly but please remain in your vehicle or at good distance until instructed accordingly.
Guests, friends and family are not permitted at sessions.
Unless otherwise stated, all sessions booked, start on the hour and are 45 minutes long.
On arrival we ask all clients to minimise their personal belongings, including phones and keys. We do not have the facilities to secure these during sessions. If you choose to leave them in your car you do so at your own risk.
You will be provided with and required to use hand gel sanitiser, both pre and post sessions and we expect high levels of personal hygiene throughout.
Any equipment we use, will be wiped down before any session taking place. If you are unsure you can request equipment can be cleaned at anytime.
2 Meter distancing will be employed throughout all sessions. This will require verbal instruction for exercise correction and adjustments. If you are unsure or require further guidance at any time, please ask.
Please ensure you bring drinking water and a hand towel to all sessions. The hand towel should be washed between use and you must remove/dispose of any water bottle.
On completion of a session I ask that you vacate the location promptly so I can transition into preparing equipment for the next session or to clean and store.
